Participation Plaques This form is to be completed by those keelboat skippers who wish to have a participation plaque produced for the 2009/10 Sailing Season. The plaque will display a record of success for the current season and be of a form which is able to be affixed to a yacht's bulkhead. A yacht must have participated in at least seven events to be eligible. Plaques will be available to skippers at the Keelboat Prize Night.	Participation Plaque Request Form
